How to Get New Ration Card/Duplicate Ration Card Haryana

Duplicate Ration Card:  When the Applicant's (Consumer) ration card has been lost or damaged, then he has to apply for Duplicate Ration Card.

How to Get Duplicate/New Ration Card
  1. Completely filled FORM D1-A will have to be submitted either in the office of District Food and Supplies Controller / Assistant Food and Supplies Officer / Inspector Food and Supplies OR In the concerned Tehsil / District Level e- Disha Centre between 9AM to 1PM in any of the working days.
  2. Applicant will submit two group photos of his family along with the Application form.
  3. The consumer will get a computer generated slip indicating the date as to when he should contact the concerned office for getting ration card.
  4. Ration card can be collected ( within 7 working days ) by the applicant from the office of District Food and Supplies Controller / Assistant Food and Supplies Officer /Inspector Food and Supplies OR concerned Tehsil / District Level e-Disha Centre where he submitted his application.

Haryana Ration Card Application fee Rs.10 /- (No Fee will be charged for BPL Families).

How to Get New Member/Members Included In the Ration Card 

For adding new member/members included in ration card, the consumer must fill the form D-I (annexure-I) the office of DFSC/AFSO/IFS in whose jurisdiction his residence falls. These application forms available in the office of DFSC/AFSO/IFS.

He will submit to attested photos passport size, of his family along with the application form.

Submit your birth certificate along with the D-I (annexure-I) and in the case of marriage the applicant will submit surrender certificate of his wife/daughter-in-law.

The consumer will get a slip indicating the date as to when he should contact the concerned office for collecting the ration card by paying the prescribed fee. This will be normally two weeks after the date of submission of the application form.

The Sub-Inspector/Inspector will verify the particulars by physical visiting the residence of the applicant and making necessary inquiries and he will also consult voter list/census record while making recommendation. After considering the recommendation, the DFSC/AFSO will issue the ration card and paste one copy of family photograph on the same.

The ration card can be collected by the applicant from the office of District Food and Supplies Controller/ Assistant Food and Supplies Officer/Inspector Food and Supplies where he submitted his application (Form D-1).
